2013 KAMM Membership Pay by Check

 

KAMM Membership is based on the calendar year.

All required fields are marked with an asterisk.  Please complete the form below.

When you hit submit, you will be sent to the PayPal’s page, however, you have registered to Pay by check.  You will receive an e-mail confirmation from KAMM.  Please print it and mail a check written to KAMM PO Box 1016, Frankfort, KY 40602-1016.  You may need to check your junk/spam e- mail folder for the KAMM auto response for registration.

If you are a registering as Agency Special, please check Agency Special below.  In the Payment Method, select ”Agency Special”.

 

Agency Special has no fee associated, it is for membership registration only and in combination with the KAMM conference registration

Please register each individual separately. However, you may want to write one check to KAMM for a group. As a result, please print the e-mail auto-confirmation for each person with payment. Additional e-addresses can be entered in the field below.
List names of registrants in the group payment. You may do this once for our records, and not for every individual that you register.
